We registered at two places. Target and Wal-Mart. My family doesn't live in a town with a Target (though, most of them won't buy us anything off the registry anyway...thus we'll end up with about 100 blankets we don't need, but that's another rant for another time), so we registered at Wal-Mart. Most of my friends won't set foot in a Wal-Mart (and live in the city), so for them we registered at Target.
We registered the weekend after we found out we were having a boy, so at 17 weeks.
I am registered at only one place, Walmart, because it is all we have locally and most of my family members do not have internet access (and would probably not be able to figure out online shopping if they did, lol). I have been picking up a lot of secondhand items from FreeCycle and Craigslist, so I have most of the major items I'll need, but I did register for a lot of smaller items.
Walmart.com's registry tips suggest registering for many items without feeling greedy - shower guests will have more options to choose from, and won't feel pressured into spending more or less than they intended to, or risk picking up an item you wouldn't necessarily like or need.
I started my registry around 4 or 5 months pregnant, though I still update it often as I acquire more items on my own or read more product reviews. I will continue to do this until my shower, which will probably be held in late January or early February, as I am due in mid-March.
Most people around me register at Babies R Us and Target. Babies R Us has everything you would possibly need but not everyone lives close to a Babies R Us or shops online so Target is a good backup. (or a similar store)
We won't be registering for a bit so I'm not sure where I will register. Target has a HORRIBLE return policy. In order to even exchange you need a gift receipt or a receipt even if you show them on your registry that you got too many of something. So I don't think I will register there. That's just me though, most people do.
I would say just to register at one place unless it a BabiesRUs or something similar. My family lives all over the place so they can't always get to a BabiesRUs very easily. Anyone who registers for any occasion always makes sure to register at Target (or similar) so they know there is one near eerybody.
It's just something to keep in mind if you have family coming from a large area.
Oh, and keep in mind that, if this is your second baby, not many people expect you to have a shower/bring presents. Even if it is a different sex. Congratulations on your little girl though!

We registered at Babies R Us and Target. I monitored my registry lists on line and added and subtracted things as needed and tried to have a variety of pricing - some more expensive items for immediate family or people who wanted to buy a group gift and loads of inexpensive stuff, too.
We registered around the 5 month point. If you are looking for specific patterns / fabrics then be careful not to register too early as this type of stuff gets discontinued and you may be left with the car seat but not the matching stroller etc . . .
I honestly started my registires when I was around 6 weeks pregnant, at that point mostly to keep the items I wanted on a saved list. I added and deleted items as needed.

If you wind up with 3 or 4 the same item and it wasn't scanned on your registry you can only exchange it for the same item in a different size or color.
